Description
A new “Assessment Tools & Questionnaires” section is now available in your Andaman7 health record. Patients and caregivers can easily complete standardized health questionnaires and evaluations directly in the PHR, with results securely shareable through your Circle of Trust or via email.
Why is this important?
Andaman7's new PHR section for patient-driven healthcare questionnaires is significant because it supports decentralized care. It enables patients and healthcare providers (HCPs) to collaboratively evaluate and monitor health using validated tools, outside traditional healthcare settings. For example, a tool based on the WHO’s ICOPE framework for assessing intrinsic capacity.
This fosters patient autonomy and stronger patient-HCP collaboration, enhances communication through secure data sharing with trusted individuals, and enriches health records with questionnaire responses. All while promoting informed care decisions and personalized health management for improved outcomes.
How does it work?
Navigate to the “Assessment tools and questionnaires” section in your own Andaman7 health record or that of the relevant patient. Click the “+” icon to select and complete a questionnaire. Once submitted, you can share the results via email using the unlocked share button.
As a core part of your Andaman7 health record, the “Assessment Tools & Questionnaires” section can be shared with your Circle of Trust as well. Note that in a shared record, anyone with access can start a questionnaire, but only the creator (who clicks the “+”) can complete it, ensuring traceability. This allows caregivers and healthcare providers (HCPs) to also use these tools on behalf of patients, such as children, the elderly, or those with cognitive challenges, supporting effective home monitoring.

This new section is available in all Andaman7 health records. From the sections’ view, tap the “More sections” tile and look for “Assessment Tools & Questionnaires” in the list.
